Operation Changes – Coronavirus (COVID-19) Due to the COVID-19 pandemic, and in accordance with the Office of the Governor, the Office of the City Clerk at Waukegan City Hall is closed to the public until further notice. However, we are busy executing the duties of the office and making accommodations to keep our employees safe as well as provide essential services. Phone calls to City Hall, emails and voicemails will be retrieved, distributed and attended to on a daily basis. The Office of the City Clerk will continue to keep the public informed regarding City Council meetings as well as prepare agendas, plus, process FOIA’s, Death & Birth certificates in addition to liquor licensing renewals during this time. Please Note: The Office of the Waukegan City Clerk only maintains birth records for a period of 10 years from the date of birth.
For birth records beyond this time period, please contact the Lake County Clerk’s Office.
Who Can Access Birth Records
Access to birth records is restricted. Birth records are available only to:
- The parent of the person named in the record. (must provide ID)
- A legal guardian who provides proof of court appointment. (must provide ID)
- A legal representative who has notarized authorization from the person named in the record or the parent. (must provide ID)
